Diabetes A to Z Series

Installment No. 2 – Type 1 Diabetes

For this installment of the Diabetes A to Z series we are going to discuss Type 1 Diabetes. This type of Diabetes was most widely known as juvenile diabetes as is usually diagnosed in children and young adults. Unlike other forms of diabetes, such as Type 2 diabetes where the body does not produce enough insulin, Type 1 diabetes is a result of the body producing no insulin. As a result the daily impacts and potential complications from this type of diabetes are much more serious and those diagnosed with this form of diabetes must be vigilant in their monitoring and management.

First Some Type 1 Diabetes Statics….

Before we get into the weeds on the specifics regarding this type of diabetes let’s revel some statics on the frequency of Type 1 versus other forms of diabetes. If you are concerned that your child is a candidate for this form of diabetes then these stats will help you rationalize the basis for your determination. On word of caution, it is always best to seek the advice of our physican as only they can confirm Type 1 diabetes. In addition, your physican can guide you through management of the diabetes if necessary.

Data compiled from the National Diabetes Fact Sheet, 2011 (Dated 1/26/11)

Total Population Impacted: 25.8 million children and adults in the United States. That is 8.3% of the population that is considered to have some form of diabetes. Although it is estimated that only 5% of the people have Type 1 Diabetes. Some more alarming facts are:

18.8 million people of the total number impacted are diagnosed

7.0 million people go undiagnosed

As many as 79 million people are prediabetic (we spoke about this topic in Installment No. 1)

Broken down by age group:

> age 65 – 10.9 million, or 26.9% of all people in this age group have diabetes

Between ages 20 and 65 – 25.6 million, or 11.3% of all people in this age group have diabetes

Under 20 – 215,000, or 0.26% of all people in this age group have diabetes. However about 1 in 400 children develop Type 1 diabetes.

Broken down by gender:

13.0 million, or 11.8% of all men aged 20 years or older have diabetes

12.6 million, or 10.8% of all women aged 20 years or older have diabetes

Type 1 Health Concerns

As with any type of diabetes we need to know the potential health issues. It is important to remember that if you have or are concerned of developing Type 1 diabetes, it does not necessarly mean that you will have the complications listed below. Proper management is the key here. Only through the guidance of your medical professional can you properly manage any diabetes condition.

Complication from Type 1 diabetes can include heart disease, stroke, high blood pressure, blindness, kidney disease, nervous system disease (neuropathy) and in some cases amputation. In fact diabetes in general was considered an underlying cause or contributing factor in more than 231,000 deaths in 2007. Here are some specifics on each of the health concerns noted

Heart disease and stroke

The risk of death for adults from heart disease is about 2 to 4 times higher than adults without diabetes.

The risk for stroke is 2 to 4 times higher among people with diabetes.

Blindness

Diabetes is the leading cause of new cases of blindness among adults aged 20–74 years.

Kidney disease

Diabetes is the leading cause of kidney failure, accounting for 44% of new cases in 2008.

In 2008, 48,374 people with diabetes began treatment for end-stage kidney disease in the United States.

In 2008, a total of 202,290 people with end-stage kidney disease due to diabetes were living on chronic dialysis or with a kidney transplant in the United States.

Nervous system disease (Neuropathy)

About 60% to 70% of people with diabetes have mild to severe forms of nervous system damage.

Amputation

More than 60% of nontraumatic lower-limb amputations occur in people with diabetes.

Now while this may cause you to pause, remember these are typical health issues with diabetes in general. So you may be asking what to do if your child has Type 1 diabetes? What can I do in raising my child to limit their potential for these health impacts. We have found a great resource for you to consider, Guide to Raising a Child with Diabetes. It will address your questions that we have not addressed in this article.

Living with Type 1 Diabetes

You have been told you have Type 1 diabetes. You feel it is the worst day of your life. I will be honest…it is not. Will it mean that you can go on with your life as you did before the diagnoses? No. But it does not mean your life as you know it has to stop either. It just means that you have to modify what and how you do some things on a daily basis. If you can get into that mindset then there will be very little that you can’t do. In fact here are some examples of people that did not let their Type 1 diabetes hold them back.

Nat Strand, diagnosed at age 12,competed in the Amazing Race on CBS and won.

Crystal Bowersox competed on American Idol

Charlie Kimball became a NASCAR driver and competes in the Indy 500

Nick Joans became a well known pop star

Go for the brass ring!!!

Glucose Monitoring & Insulin

Sorry, forgot to mention how best to manage Type 1 diabetes. It is basically involves a few elements: blood glucose control, insulin management, exercise, nutrition and support.

First is glucose control. Since your body is no longer producing insulin you will need to learn both how much daily insulin is needed and how best to administer. The monitoring is pretty straight forward. You will need to take blood samples and do a blood glucose level test several times a day. Once you have determined your glucose level you can administer the necessary insulin by either injection pens, syringes or an insulin pump. As we have stated previously it is important to work with your health professional to determine which insulin regimen is best for you. If you need supplies, check out our library page for a complete list of links that will connect you to major suppliers of these products at a nice discount.

Exercise

Exercise provides some of the best benefits as it is one of the key components of proper diabetes care. Beside all of the benefits we normally associate with exercise, your blood will maintain more stable glucose levels. Now this does not mean that you have to run a marathon. Let’s face it, most of us could not run a marathon if we wanted…I know I can’t. All you need to do is stay active doing the things you may already be doing. However if you need some ideas try some of these suggestions:

Go for a walk with your wife or husband, discuss how your day went. Not only will you both get exercise, you can de-stress by getting work issues off your chest.

How about going to the gym? If you already do this as part of your health routine then great, you are ahead of the game. Strength is not only great for diabetes but can counteract many issues we face as we get older.

Here is one for the parents/grandparents…. play with the kids. See who says this can’t be fun. Those with kids/grandchildren know how tiring playing with the kids can be. Now if that is not exercise I do not know what is.

Take the stairs instead of the elevator

Go dancing. Make a night out.

Walk around the house while on the phone. I know I catch myself doing this sometimes.

Walk every aisle when you go to the store.

There are endless possibilities.

Nutrition

As nutrition relates directly to your glucose level, it is a very important part of managing your diabetes. It is important to develop a meal plan that properly deals with managing blood glucose levels. We have mentioned in the past several resources for both managing weight and glucose levels. Click on any of the links below for the specific meal plans that meet your needs.

Support

One aspect often forgotten is support. Support plays an important role in diabetes care. Being able to connect with other people who understand the daily issues that must be faced, both emotional and physical can make all the difference. There are many organization available in every community that can provide the support you need. If you can ‘t find any support groups nearby than try the internet. In fact the internet can provide support on a 24/7 basis that a live group can not.

All the Best…..

Diabetes A to Z Series

Installment No. 1 – Prediabetes

As we begin our series on Diabetes and the different type and impacts it can have on ones way of life, there is no better place to start than prediabetes. In fact if you are visiting this site for the first time you are probably here because you or someone you care about may be wondering if they are in this category and need to find out more information. Well, you have come to the right place as we are going to address this issue in detail….so let’s begin…

Prediabetes Defined

For those people that have developed some form of diabetes, mostly those that have type 2 diabetes, they were most likely prediabetic and may not have even known it. As is the case with diabetes, it is the level of glucose in the blood that determines this condition. Usually the blood glucose levels are higher than normal but do not typically diagnosed as diabetes (again mostly type 2 diabetes). If you are thinking how can this be…if the medical profession is aware of this condition, why does it not get identified? Seems logical but in actuality it can be easily missed. In fact it is estimated by the American Diabetes Association that there are 79 million people in the US that go undiagnosed with this condition. Think of it this way, have you ever taken your car into the dealership for a noise only to get there and not hear the noise? I know that has happened to me on more than one occasion. Well it is similar to identifying a higher than normal blood glucose level. You may not have a higher level at the time you visit your doctor but did so just a couple days earlier and as a result it gets missed. One way to better detect if you are prediabetic is to monitor and record your own blood glucose levels at regular intervals. Provide this information to your medical professional for review. Remember early detection is the key here, especially if you have a family history of diabetes or are experiencing symptoms that could be attributed to diabetes. As the saying goes….no harm…no foul.

Prediabetes…..Symptoms, Health Implications??

Some of the symptoms that may indicate prediabetes are the following:

Frequent infections of the gum, bladder or skin

Feeling fatigued

Impaired vision

Constant or unusual thirst

Loss of feeling in the hands/feet

Frequent urination

Bruises/cuts that heal slow

As we noted previously there is a greater chance of developing diabetes and in particular type 2 diabetes if you are prediabetic. Studies by the Diabetes Prevention Program indicated that every year 11% of the people with prediabetes developed type 2 diabetes during three years of follow-up. However there have been other studies that indicate that it could be as long as 10 years. It depends ultimately on what steps are taken to manage your blood glucose levels. This is supported by additional research that indicates that long term damage to the heart and circulatory system may occur with being prediabetic.
What is My Prediabetes Risk? Who should be Tested?

There are three main tests to determine if your metabolism is normal, pre-diabetic or diabetic.

Fasting Plasma Glucose Test (FPG) – Fasting overnight w/ glucose testing prior to eating

o <100mg/dl (normal), =100mg/dl <126mg/dl (prediabetic), =126mg/dl (diabetic)

AIC Test – average range of glucose over the past 3-4 months

o <5,7%(normal), =5.7% <6.5%(prediabetic), = 6.5%(diabetic)

Oral Glucose Tolerance Test (OGTT) – Fasting overnight w/ testing first thing in the morning & 2 hours after consuming a glucose rich drink

o <140mg/dl(normal), =140mg/dl <200mg/dl(prediabetic), =200mg/dl(diabetic)

An important point to make here is that if an OGTT test reveals an abnormal blood glucose level you are considered to have impaired glucose tolerance (IGT). For an abnormal glucose level on a FGP test, you are considered to have impaired fasting glucose (IFG). While these may have different medical terms they both indicate the same condition….prediabetes.

If you want to find out your risk level, check out the Diabetes Risk Test. There you can print out a free form that you can fill out to better determine if you need one of the previous tests mentioned. A high score may suggest that additional testing is necessary. Find out your score!

One final note on testing for prediabetes. While this condition occurs in all races and age groups, African Americans, Latinos, Asian Americans/Pacific Islanders and Native Americans are at a higher risk of developing this condition. However if you are 45 or older and overweight you should be checked at your next regular visit. If you are under the age of 45 but have high blood pressure, low HDL cholesterol and high triglycerides, a family history of gestational diabetes or diabetes, you should also be tested at your next checkup.

Prediabetes Confirmed…Can it be Treated?

As we stated before, it depends on how the individual chooses to manage their blood glucose levels that determines any success of treatment. The good news is that prediabetes is not diabetes and does not have many of the medical impacts as diabetes. So the goal is to keep it that way and one of the best ways to that is make changes in diet and increase the amount of exercise. A study by the Diabetes Prevention Program (DPP) indicated that 30 minutes of moderate physical exercise and a reduction of 7% in body weight resulted in a 58% reduced risk to developing type 2 diabetes over medication alone.

Now if you are saying …7% reduction in body weight!!! Well not to worry as we have many resources on this site that can assist you in reaching this goal. Check out just some of the great resources we have compiled on this topic.
Gestational Diabetes – What is it, How does it develop?

As indicated by the word Diabetes, you might have guessed that this is a condition involving a higher than normal blood sugar level (glucose) but what does it mean to be Gestational Diabetes? Well this condition is actually related to developing Diabetes during pregnancy. Approximately 4% of women develop this form of Diabetes during pregnancy. Although all women will have some type of impaired glucose intolerance as a result of the hormonal changes that their body will be going through, their higher glucose levels are not usually serious enough for the development of Diabetes. However for some women the hormonal changes as a result of the pregnancy places them at risk for developing Diabetes during their third trimester.

The cause is related to the placenta and the increased levels of certain hormones that are produced to actually help prevent the mother from developing low blood sugar. In essence these hormones produced help stop the actions of the naturally produced insulin. Since insulin regulates the level of blood sugar in the system, the hormones produced by the placenta counteract the insulin and result in a high blood sugar level. Over the course of a pregnancy progressive impaired intolerance is developed. The body tries to lower the blood sugar level by producing more insulin from the pancreas, however if the pancreas can not produce enough insulin to overcome the effect of the pregnancy hormone levels then there is a good chance that Gestational Diabetes will develop.

Although gestational diabetes may seem like a cruel joke from the big guy upstairs, there is some good news. Most women return to normal glucose levels within about 6 weeks after the birth of their child and the diabetes is no longer an immediate factor. You may need to take another test several weeks later to confirm but this is usually not required. However you are not out of the woods yet as women that develop Diabetes during pregnancy have a 60% greater risk of developing type 2 Diabetes later in life. So if you find yourself in this group it will be important for you to follow a healthy diet, exercise and maintain an ideal body weight to help reduce your risk.

Top risk Factors for Gestational Diabetes during pregnancy

Family history of Diabetes

Previous pregnancy involving a stillborn or baby over 9 pounds

20% or more overweight prior to pregnancy

Member of a high risk group (Asian, Black, Native American, Hispanic)

Gestational Diabetes in a prior pregnancy

Previous impaired glucose levels (higher than normal glucose levels but no Diabetes)

Excessive amniotic fluid (called polyhdramnios)

Sugar in the urine

It is important to note that although these are the top risk factors many women that develop gestational diabetes have none of the top risk factors.

How I know if I have Gestational Diabetes?

The key is to first asses if you have a high risk for gestational diabetes. If you have some of the high risk factor previously mentioned then you should be tested as early as possible during your pregnancy. If you do not have any of the high risk factors then you should be tested between the 24th and 28th week of pregnancy. The important thing here to to be sure and discuss this issue with your doctor if you have a concern as there is no one solution for every person

The test for gestational diabetes is easy and relatively quick. It is called an oral glucose tolerance test and involves consuming a sweet liquid that is absorbed by the body quickly and causes the blood sugar levels to rise within 1 hours time. After an hour the blood is tested and a blood sugar level greater than or equal to 140 mg/dL is considered to be abnormal and additional test may be required to verify development risk.

What Are The Risk Having Gestational Diabetes?

Gestational Diabetes can affect the development of the baby throughout pregnancy. If developed early in the pregnancy there is the risk of birth defects that affect the major organs as well as the brain. In addition, there is an increased risk of miscarriage. In the second and third trimester of pregnancy the diabetes can result in over nutrition to the baby and cause the baby to be larger than normal. Now while this may not seem to be a concern on the surface, a larger baby can cause additional trauma to the mother and shoulder trauma to the baby if delivered naturally, especially for smaller women. Most often this situation requires a caesarean type delivery. Finally a baby once delivered can suffer from hyperinsulinemia. A condition in which the blood sugar level of the baby drops to very low levels as they are no longer receiving high blood sugar levels from the mother. While this may seems frighting, it is important to remember that just because you may be at risk does not mean that you will experience any of these issues. With proper treatment there is no reason why you can not deliver a healthy baby. Trust you physician and if you do not understand…ASK QUESTIONS!

Ok I Have Gestational Diabetes…… Now What?

Managing Your Gestational Diabetes

Keep your high blood pressure under control

Monitor your blood sugar levels at least several times during the day (this may include monitoring urine for Keytones)

Keep a journal of monitoring results, glucose test, diet, exercise and general overall how you feel

Exercise as allowed by your physician

Diet Changes with Gestational Diabetes

While some of the steps below may take some effort on your part, they will go a long way towards you and your babies health.

Drink 64oz of liquids per day (min)

Ensure your diet includes sufficient vitamins and minerals. Consult your health physician regarding the use prenatal supplements to meet these needs

Consume foods high in fiber (20-35 grams per day)

Hey everyone, hope you are enjoying the Diabetics Diet Guide we told you about recently. I have gotten some really good feedback on the book, one from my father in law who could not believe how well his blood sugar was being managed. It appears that this is a hit. He is looking forward to his next visit to the doctors and hope he may be able to reduce some of his medication. Now I do not know what exactly he takes but I do know this….when he takes his pills it looks like a handful of M&M’s in the palm of his hand. In future updates I will let you know the result of his visit.

Part of the reason for this post is a result of my father in law. See he was impressed with my recommendation of the Diabetics Diet Guide that he asked me to do some additional research to find some good sources on Diabetic cooking. After searching for the past couple of weeks and reading endless reviews, I have compiled a list of cooking resources that you may want to consider as a compliment to the Diabetics Diet Guide . I have listed their links below for your review and consideration. Remember these titles will also be listed in the categories section on the left side of the page so don’t worry if you come back at a later date.

Best Diabetes Diets- Diabetics Diet Guide

Registered Nurse, Veteran Nurse, Medical Researcher, Health Consultant & Author Anissa Preston, BSN, RN has developed a Diabetics Diet Guide specifically targeted to allow diabetics to control their diabetes without medications and without any painful side effects.

As a result of years of experience as a registered nurse and experiencing diabetes herself, she found that explaining the importance of diabetes was not the problem. It was recommending an actual diet that was a big problem. See there a lots of books on diabetes but it became tough for patients to find a diabetes reference that had any focus on the diabetic diet if at all. As she struggled to manage her own diabetes, she decided to develop her own guide so with the support of doctors that she has worked with for many years. With hundreds of hours of researching and writing she developed the Diabetics Diet Guide

In case you missed it….YES…she is not only a nurse but also has diabetes herself!!!

What a great source. Someone that has actually used the guide suggestions. You can not beat that!!

In using her guide book, she will teach you how to reverse fatigue, eliminate pain, manage your blood sugar levels and reduce your overall diabetes symptoms.

Have you ever asked yourself the following questions?

Am I at risk for Diabetes

What if I am Diabetic…what can I eat

Is there anything I can do to stop Diabetes

What are the best foods

Does it matter what time I eat…if so what is the best time

I have been told that exercise will help fight my Diabetes, but which exercises are best

I am pregnant and a Diabetic, what foods should I eat

What are the misconceptions to look out for regarding Diabetes Diets

Diabetics Diet Guide has the answers to these as well as many other questions. In fact you will also learn things that your doctor probably has not told you about Diabetes.

If you have Diabetes or believe you are developing Diabetes, there is one important thing you need to know……

Dear Nurse Anissa,

I have been a diabetic for 16 years and I have never felt this good in my entire life. Just after 2 weeks of following your diabetes diet guide ebook, my fatigue level dropped to about 40%!

Its really amazing how this much information was kept from me by my doctors. And mind you, I already talked to about a dozen doctors even before my wife and I moved to Stockton, CA.

I made it a pact to myself to send you an email about the results.

If you don’t mind, I am sharing your ebook to a friend of mine as im sure he needs it.

Thanks a whole lot!

    var s = document.getElementsByTagName('script')[0]; s.parentNode.insertBefore(ga, s);
  })();

It is no secret that the epidemic of obesity has been an issue for several years and it only seems to be getting worse. The rates of childhood obesity are rising and the link between diabetes and diet and the state of our health cannot be overstated. One very common problem that can arise from being overweight is diabetes. This is a condition of the body being unable to efficiently process the amount of sugar that is in the bloodstream and the result of this constant barrage of sugar without the proper natural insulin levels to counteract it can give rise to many serious health complications. We discuss these potential health issues later in the article.

How can we watch our diet and eat properly so as to do what we can to prevent the onset of diabetes? One thing that seems obvious is to avoid things that are high in sugars. The usual categories of candies and desserts are some types of food to avoid, but did you know that carbohydrates that are found in many so called “white foods” are also a problem for the body to process?

Foods that are “white” are generally the refined carbohydrates and those can be bad for the health of the person who has diabetes. White bread, refined sugars and white rice are examples of carbohydrates that are quickly converted by the body into sugars, which can aggravate the problem for someone whose system is not handling the sugar levels well.

The best sort of diet to eat would be one that is containing a moderate level of complex carbohydrates and very little of the more simple sugar type foods. Eating fiber is important because it can aid in feeling fuller faster, thus reducing the total amount of calories consumed.

A diet that is high in fresh vegetables and whole grains with lean meats can do a lot to reduce the blood sugars and to help avoid or improve diabetes. Portion control is always important and the overall caloric intake is something that must be carefully monitored as well in order to keep health good and the body in balance for the long term.

So you may now be asking what kind of health issues can result for developing diabetes? One of the more serious issues is renal disease and kidney failure. For renal disease, diabetes is the number one cause and in fact kidney disease as a result of diabetes is responsible for approximately 44% of all kidney failure cases. The reason for this is that due to the excess buildup of glucose in the bloodstream and the inability of the body to manage it, a specific protein in the blood (called glomeruli) becomes damaged and is passed through to the kidney and leak out in the urine. Over time the kidney is pressured to try to hold onto more of this protein but is overworked and can not. In the end the kidney shuts down and forces the individual to choose between a kidney transplant or dialysis.

However you now can take steps to control or even halt the progression of this disease through diet. As we noted in a previous article, Top 10 Tips, there are ways to manage this disease. One of the best ways is to get educated on diet options. The best source that we have found on this subject is the Diabetics Diet Guide. The first reference devoted entirely to the diabetic diet. It is impressive and we strongly suggest that you make it a part of your reference materials.

Best of luck
